Funding Request Terms and Conditions

By submitting a funding request, you are agreeing to the following:
The learner’s name and email acting as an electronic signature of the learner to commit to all the terms and conditions.
You confirm your commitment to completing the education opportunity listed in the submitted form.
You confirm you have discussed and agreed the education opportunity with your line-manager, and that your Practice or Primary Care Network (PCN) supports your training.
Your Practice / PCN will nominate a designated supervisor to be responsible for the provision of all required support and to be reasonably available throughout your training.
The Practice / PCN will release you to attend study days and any other educational commitments (e.g. exams, reflective learning sessions etc.) throughout the duration of the programme.
On completion of your learning, you will provide proof of attendance and participate in evaluations that evidence return on investment.
You will inform NATH immediately if your studies cease.
You understand that failing to attend the course, or providing proof of attendance, may result in your employer being billed for your place.
You confirm that you have read and understood the NATH Privacy Notice and agree to the processing of your personal data by NATH for the purposes outlined in the funding form.
