Reflections from the Peak Health Coaching Annual Conference 

Darcie Parry, Our Care Coordinator Lead, was invited to attend and contribute to the Care Coordinator Annual Conference hosted by Peak Health Coaching. The event brought together Care Coordinators from across the country, creating a unique space to connect, reflect and explore the evolving role within Primary Care. 
 
With over 127 Care Coordinators attending, the conference offered a valuable opportunity to share experiences, learn from one another and build a stronger understanding of how the role is being delivered across different settings.

🀝 Contributing to the Conference 

Following the initial invitation, Darcie connected with the Peak Health Coaching team to explore how she could support the event. As part of the programme, she delivered a 25-minute presentation focusing on the Care Coordinator role, including what the role involves, how it is developing across Primary Care, and the value it brings to patient care. 
 
This was followed by a 25-minute breakout session centred on career development, training opportunities and apprenticeships. During this session, Darcie also introduced the role of training hubs, highlighting how they can support Care Coordinators both locally and nationally with access to learning, development and progression pathways. 
 
The session saw strong engagement, with over 50 attendees joining to explore development opportunities and ask questions. 

🎯 Engagement and Meaningful Conversations 

The conference created a highly interactive and supportive environment. Throughout both the main session and breakout discussions, Care Coordinators actively engaged through chat, shared reflections and asked thoughtful questions about their roles and future pathways. 
 
Darcie connected with many attendees during the sessions, with conversations focusing on real-life experiences, challenges and opportunities within the role. Following the conference, four Care Coordinators reached out directly for further support, particularly around how to connect with their local training hubs and access development opportunities. 
 
An open invitation was also shared at the end of the session for anyone seeking additional guidance, reinforcing the importance of ongoing support beyond the event itself. 

πŸ’‘ Key Themes and Insights from the Day 

The conference highlighted several important themes that reflect both the strengths and ongoing development of the Care Coordinator role. 
 
One of the most significant outcomes was an improved understanding of the role itself. Attendees shared that they left with greater clarity on what Care Coordinators do, how the role fits within Primary Care, and how it contributes to patient outcomes. 
 
There was also strong recognition that Care Coordinator roles can look different across organisations. Despite this variation, a shared purpose was evident. At the heart of every role is the aim to support patients through personalised care, ensuring individuals receive the right support at the right time. 
 
The concept of humanising care was a central theme throughout the conference. Discussions highlighted the importance of seeing patients beyond their conditions, focusing on their individual needs, preferences and circumstances. This reinforced the value of Care Coordinators in delivering truly personalised care. 
 
Another key learning point was the opportunity to reflect on how the role is being used in practice. Attendees openly discussed where the role is working well and where there may be misunderstandings or inconsistencies. These conversations were valuable in identifying areas for improvement and shared learning. 
 
Training and development emerged as a strong area of interest. Many Care Coordinators were keen to understand how they could access further training, develop their skills and progress within their careers. The breakout session provided a space to explore these opportunities in more detail and highlight available support. 

🌱 Supporting Ongoing Development 

A key outcome from the conference was the continuation of support beyond the event itself. The level of engagement demonstrated a clear need for accessible guidance, structured development pathways and ongoing connection. 
 
The follow-up conversations with Care Coordinators highlighted the importance of signposting to training hubs and ensuring individuals know where to go for support. This ongoing engagement helps to bridge gaps in knowledge and ensures that Care Coordinators feel supported in their roles.

The conference was a highly positive experience, with encouraging feedback shared with the Peak Health Coaching team. It created a welcoming and collaborative environment where Care Coordinators felt comfortable sharing their experiences and learning from one another. 
 
From Darcie’s perspective, it was an invaluable opportunity to connect directly with the workforce, understand their needs and contribute to their development in a meaningful way. The level of enthusiasm, openness and willingness to learn was particularly encouraging.

The Care Coordinator role continues to grow and evolve within Primary Care. Events like this play an important role in strengthening understanding, building confidence and supporting consistency across the system. 
 
By bringing people together, encouraging open conversations and highlighting development opportunities, conferences like this help shape the future of the role and the impact it has on patient care.
