Hollie Bloom Shortlisted for the General Practice Awards 2025
We are delighted to announce that Hollie Bloom, one of our dedicated General Practice Nurse (GPN) Mid-Career Fellows, has been shortlisted for the General Practice Awards 2025. Her Quality Improvement (QI) project, “Housebound Nurse Service: Holistic Home-Based Care for People with Long-Term Conditions”, has been recognised in the prestigious Clinical Improvement – Long Term Conditions category.
This achievement reflects not only Hollie’s passion for patient-centred care, but also the importance of supporting innovation within general practice nursing.
About the Project: Housebound Nurse Service
The Housebound Nurse Service was developed in response to a pressing need within primary care: ensuring equitable access to care for patients living with long-term conditions who are unable to attend healthcare settings in person.
Hollie’s project focuses on creating a holistic, home-based model of care, where nurses provide regular visits to patients in their own homes. This service is designed to:
Deliver continuity of care for housebound patients who would otherwise struggle to access routine reviews and ongoing management.
Offer holistic support, addressing both the medical and wider wellbeing needs of patients.
Improve patient safety, by ensuring timely monitoring, medication reviews, and care adjustments.
Reduce inequalities in healthcare, particularly for vulnerable patients at higher risk of isolation and poorer outcomes.
By taking services directly to patients’ homes, Hollie’s project demonstrates how nursing-led innovation can transform the way care is delivered and ensure that no one is left behind.
Why This Matters
Patients living with long-term conditions often require ongoing support to manage their health effectively. For those who are housebound, challenges such as limited mobility, lack of transport, or social isolation can make it difficult to access the care they need.
Without tailored services, these patients may experience gaps in their care, leading to avoidable complications, hospital admissions, or declines in quality of life. Hollie’s work highlights how proactive, community-based nursing can make a significant difference by:

The Fellowship: Supporting Nursing Leadership
Hollie’s success is a reflection of the opportunities provided through the GPN Mid-Career Fellowship.
