REACT TO Home Rounds delivered
NATH has successfully completed its REACT TO Home Rounds training across 54 care homes.
The face-to-face training, delivered by Michele Tuttle, was designed to increase the effectiveness of home rounds, giving staff the skills and confidence to understand which residents to refer, improved preparation and follow-up, and increased staff awareness of what support is available for local health care services.
The training aims to reinforce existing knowledge, while opening up new ways of working to improve collaboration with other primary care professionals.
